LCA Feedback from recent investigations

LIKE/WISH   (0-5, 5 HIGH)

I LIKED THIS PROCESS.  IT IS THE 3RD MAXI WE HAVE DONE AND IT WAS FAR BETTER THAN THE OTHERS BECAUSE PARTICIPANTS HAD THE OPPORTUNITY TO CONNECT THE DOTS.  IT HELPS BUILD RESPECT AND RELEASE BLAME!   (5)

I LIKE THAT WE ALL KNOW WHAT OUR LATENT ISSUE IS AND THIS PROCESS FINALLY ALLOWED US TO BRING IT UP AND ADMIT IT.  THIS WILL HELP US ADDRESS IT.  (5)

I LIKED HOW WE DISSECTED THE WHOLE INCIDENT FROM BOTTOM TO TOP TO GET A BETTER UNDERSTANDING OF EVERYTHING AND EVERYONE INVOLVED. (5)

I LIKE THE RESULTS OF ALL THE ACTION ITEMS THAT CAME OUT OF THIS FOR THE HELP OF SAFETY AND NO FINGER POINTING! (5)

I LIKE THE WAY THIS PROCESS REMOVED BLAME AND REVEALED HOW OUR ENTIRE GROUP AND INDIVIDUAL ACTIONS AFFECTS THE ACTIONS OF ONE ANOTHER, YET GIVING US THE OPPORTUNITY TO BECOME ACCOUNTABLE TO EACH OTHER AND DEVELOP WAYS TO HEAL THE WOUNDS AND CORRECT PROBLEMS, IN A WAY THAT DOESN’T INFLICT MORE HARM TO INDIVIDUALS.  WE ARE ABLE TO UNDERSTAND THE TRUTH.  (5)

I NEED TO LEAD BY EXAMPLE, IN ATTITUDE AND ACTIONS.  I DON’T TAKE THE TIME TO DO THINGS SAFELY AND I NEED TO STOP OTHERS WHEN I SEE THEM DOING SOMETHING UNSAFE.  I NEED TO FOLLOW UP ON SMALL PROBLEMS AND ADDRESS THEM WITH HIGH PRIORITY: NOT LEAVING THEM FOR SOMEONE ELSE TO FIX.  (4+)

I LIKED THE PROCESS BECAUSE IT IS TRULY EYE OPENING TO HOW OTHERS SEE WHAT IS GOING ON AND HOW IT HAS CHANGED THE WAY I WILL LOOK AT THINGS.  (4)

I LIKED THE SFO PROCESS (DIDN’T DO ON PREVIOUS RCA’S) AND FELT OUTCOME WAS RIGHT ON.  I HOPE THIS IS SUPPORTED. (5)

I WISH THAT MORE PEOPLE WHO WERE INCLUDED WITH THE SITUATION WOULD HAVE ATTENDED THIS MEETING.  MORE OPERATORS, MAINTENANCE, PROCESS FOREMAN, ETC. (5)

I LIKE THE PROCESS.  AS I KNEW PREVIOUSLY IT WOULD GET TO THE CORE CAUSE OF OUR FAILURES AND DEFICIENCIES!  THE GROUP NOW MUST EMBRACE THE FINDINGS AND BUY IN TO A NEW PHILOSOPHY.  (4.5)

I LIKE HOW THE PROCESS LEADS THE STAKEHOLDERS TO THE SAME PLACE AT THE END, AFTER THEY REVIEW THE EVIDENCE.  EVERYBODY FELT THEY WOULD HAVE DONE THE SAME THINGS THAT THE OPERATOR WENT THROUGH.  (5)

I WISH EVERYONE AT THE PLANT COULD EXPERIENCE THIS PROCESS FROM START TO FINISH.  (4)

I LIKED THE TRUST AND HONESTY THAT CAME FROM THE GROUP AND THE SENSE OF CREDIBILITY THAT WAS DEVELOPED FOR ALL DISCIPLINES AND MANAGEMENT.  (4)

I WISH EVERY COMPANY EMPLOYEE COULD ATTEND THE FOUR DAY TRAINING COURSE.  THIS IS ESSENTIAL.  TO GET THE MOST OUT OF THIS PROCESS, EACH EMPLOYEE NEEDS TO ATTEND THE CLASS AND AT LEAST ONE MAXI INVESTIGATION.  FOR ME THIS INVESTIGATION ADDED TO THE CLASS MATERIAL.  (5)

I WISH EVERYONE HAD THE OPPORTUNITY TO GO THROUGH THIS PROCESS.  (5)

 

Maxi LCA Stakeholder Comments

I believe this investigation was conducted very professionally, and was successful at reaching the desired outcomes.  Also, I believe conducting the stakeholder meeting on-site was not a good choice.  Excellent job by all presenters to keep the meeting to 1 day.  Rated 5 

The investigation was done and presented unbiased.  I felt it did not try to break anyone apart or have a personal agenda.  The honesty that came out in the end was amazing.  Rated 4 

I feel that we learned a lot about the process (and investigation) showed again that little things cause big problems.  Rated 4 

I feel the investigation was done well.  The time gap from incident to stakeholder meeting was poor.  Rated 4 

It was difficult for me to get to latency because the human causes focused mainly on the events from the past dealing with people who are no longer here and which I knew little of.  I think we did get to the right latencies for how we are now.  Rated 4 

This is a very enlightening investigation, very informative and brutally honest.  Information presented was very detailed, factual and presented with open discussion allowed and encouraged at all times.  I loved the part where all present confessed to something that could be done better.  Rated 5 

I feel the investigation of the fire exposed some serious gaps of understanding with our own safety equipment.  The investigation had a clear and concrete result which will help us operate this plant more safely in the future.  Rated 5 

The content of the investigation was very good.  The process is a must up to latency.  This process needs to be done on every incident (that require it).  The info needs to be communicated.  I still do not understand the benefit of naming functions!! (In human causes)  Rated 4 

The investigation was done well.  The discussions were great.  I don’t think that completing the session in one day took away from the overall outcome.  There were many obstacles in completing the stakeholder meeting but overall it went well.  Rated 4 

This is the third major investigation I have been involved with and I struggle with how much value is gained by continually rehashing the way things were in the past.  Rating 4 

Thorough investigation, well led and seemingly well received.  Rated 5 

I feel that investigation process is valuable in breaking down existing barriers through finding latent causes rather than simple root causes.  This particular investigation helped to further identify latencies discovered in previous investigations.  I only hope that we can break through and turn this ship around.  Rated 4 

This investigation was an eye opening experience.  I have learned a lot from participating on this investigation team, and from that I have gained a much higher regard for the importance of the roles and responsibilities of all the other disciplines.  I’ve learned that we couldn’t function without each other.  Rate 5
