Investigator Experience - 2 days A VERY interactive, hands-on session that will give Principal Investigators EXPERIENCE in leading Maxi and Midi-LCA's. This seminar has been requested for years, and has been developed by two of Failsafe's Licensed and most experienced Principal Investigators: Rob Statham and Spencer Philo -- both which receive rave reviews for their leadership and teaching skills.
How to Think Differently about Small Failures - 1 day Mini LCA - 4 hours
The training takes 4 hours with a
class maximum of 30. We regularly train 2 classes in one day which is very
cost effective for your company. Anyone who attends this training will be
challenged and equipped to perform Mini LCA's
at their work site. These small efficient investigations take less time
than Maxi or Midi LCA's yet reveal the very same latent causes that contribute
to larger problems. Implementing Mini LCA's is the most effective way to
identify and address latency before you experience catastrophic failures.

Cause Analysis should be something that EVERYONE eventually does "in their
minds." Contrary to what you might think, you are probably NOT doing Latent
Cause Analysis right now. True Latent Cause Analysis requires the inquirer to
look at THEMSELVES, asking "what is it about the way I am that contributed to
this incident."
