Latent Cause Analysis - Training


The Principal Investigator Experience - 2 days

A VERY interactive, hands-on session that will give Principal Investigators EXPERIENCE in leading Maxi and Midi-LCA's. 

This seminar has been requested for years, and has been developed by two of Failsafe's Licensed and most experienced Principal Investigators:  Rob Statham and Spencer Philo -- both which receive rave reviews for their leadership and teaching skills. 

 

How to Think Differently about Small Failures - 1 day

Mini LCA - 4 hours 

Latent Cause Analysis should be something that EVERYONE eventually does "in their minds." Contrary to what you might think, you are probably NOT doing Latent Cause Analysis right now. True Latent Cause Analysis requires the inquirer to look at THEMSELVES, asking "what is it about the way I am that contributed to this incident."

This 4 hour live and interactive session will take the attendee through the Mini-LCA process:

a process designed for EVERYONE in an organization
to be used on ANY problem (preferably a SMALL problem)
to quickly gather a LOT of information about "latency"
so that the latencies can be addressed PRIOR to causing a big problem.
Attendees will be taught:

Why Latent Cause Analysis must be for everyone.
The difference between a Maxi, Midi, and Mini-LCA
The difference between Physical, Human, and Latent Causes
The 3 forms of Evidence (3Ps) to always consider
How to do a Mini-LCA
Attendees will be taken through the process in a step-by-step manner, and will perform an actual Mini-LCA on a real chemical plant. Plenty of opportunity will be available to interact with one another, sharing one another's reactions to the training.

The training takes 4 hours with a class maximum of 30.  We regularly train 2 classes in one day which is very cost effective for your company.  Anyone who attends this training will be challenged and equipped to perform Mini LCA's at their work site.  These small efficient investigations take less time than Maxi or Midi LCA's yet reveal the very same latent causes that contribute to larger problems.  Implementing Mini LCA's is the most effective way to identify and address latency before you experience catastrophic failures.  Please read class attendees comments by clicking here.
