Failures are costly, time-consuming, and inevitable.  The key to avoiding failures is to learn from past failures.  Latent Cause Analysis performed properly will teach us the hidden lessons from failures - latent causes.  The latent causes from "normal" or "nagging" failures are the very same forces at work in the big catastrophic failures.  To avoid these failures companies routinely perform RCA's on key failures and proactively address latencies before a catastrophic failure.

If your investigations end with recommendations to "re-train" then we are confident the problem has not been solved.  The symptoms have been treated, but with ever-increasing work loads and fewer resources, the PROBLEMS need to be SOLVED.  To do this you must reveal the hidden forces at work causing the failure.  The LCA process is not used to assign blame, an investigation MUST go beyond human causes to reveal the Latent causes.  Effective Latent Cause Analysis will do just this, thus avoiding multiple failures in the future -  saving time, resources, and money.

To avoid incidents in the future we need to understand why people do what they do.  Until we understand we will never change...
